Important forms for librarian reappointment/promotion/tenure.

During your 2nd and 4th year reappointments, promotion, and tenure evaluations you must fill out and include these forms in your packet of submitted materials:

Appendix A-2: Professional Activities and Responsibilities – Librarians
Check off within categories I (Continuing Scholarship) and II (Professional Activities), and within category III (Alternative Assignment) if applicable, the activity or activities in which you have engaged and provide supportive materials evidencing them. You only need to check 1 item under categories 1 and 2.
Appendix B: Comprehensive Resume
This form applies to both faculty and librarians.

The Appendix E-1(a) form below is filled out by the Library Program Area Chair and a similar one, Appendix E-1(b), is filled out by your Peer Evaluation Committee (PEC) when you have a PEC. While you don't fill this form out, it can serve you as an outline for the 4 areas you need to make sure to address in your narratives and packets.
